INFORMED CONSENT REQUIRED — By proceeding with Everin's telehealth services, you confirm that you have read, understood, and voluntarily consent to the terms described in this document. This consent must be provided individually and cannot be pre-filled on your behalf.

What Is Telehealth

Telehealth refers to the delivery of healthcare services using electronic communications, including asynchronous (store-and-forward) review of health information submitted through secure online intake forms. Through the Everin platform, independent licensed physicians review your health intake and, when clinically appropriate, issue prescriptions for compounded medications.

Telehealth services are not a substitute for in-person medical care. Some conditions cannot be safely evaluated or treated via telehealth.

Nature of Telehealth Services

You understand and acknowledge that:

Telehealth services involve the electronic transmission of your health information to an independent licensed physician
The reviewing physician will not conduct an in-person physical examination
Clinical decisions are made based solely on the information you provide in your intake form
The accuracy and completeness of the information you provide is essential to your safety
Not all conditions can be appropriately evaluated or treated through telehealth

Benefits of Telehealth

Telehealth services may offer the following benefits:

Convenient access to physician-guided care from your home
Reduced need for in-person clinic visits for eligible services
Timely physician review — typically within 48 hours
Direct-to-patient delivery of prescribed medications

Risks of Telehealth

You understand and accept the following potential risks associated with telehealth services:

Information transmitted electronically may be incomplete or misinterpreted without in-person examination
Technical failures or delays may affect the delivery of services
Security protocols may not prevent all unauthorized access to personal health information
A prescription may not be issued if telehealth is deemed clinically inappropriate for your situation

Independent Physician Services

You understand that all medical services provided through the Everin platform are delivered by independent licensed healthcare professionals. Everin is not a medical provider and does not employ, supervise, or control the physicians who review your intake. No doctor-patient relationship exists between you and Everin.

The physician's decision to issue or withhold a prescription is made solely at their clinical discretion and is final.

Privacy & Security

Your health information is protected under HIPAA and handled in accordance with our HIPAA Notice of Privacy Practices. All data transmitted through the Everin platform is encrypted using SSL and stored in HIPAA-compliant systems. See our Privacy Policy and HIPAA Notice for full details.

Emergency Care

Everin's telehealth services are not designed for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use the Everin platform or communication channels to seek emergency care.

Right to Withdraw Consent

You have the right to withdraw your consent to telehealth services at any time by contacting us at info@tryeverin.com. Withdrawal of consent will not affect care already provided. If you withdraw consent, you may no longer be able to access Everin's telehealth services.

09.
Prescription Rights

If a prescription is issued following your telehealth consultation, you have the right to transfer that prescription to any licensed pharmacy at any time at no charge. Everin will never restrict or charge for prescription transfers.
